FMA/CNA Safety Awards
Safety is a high priority for FMA and TPA members and we want to recognize their efforts. Participating in FMA's Safety Award Program is a great way to stimulate interest in accident prevention and promote safety in the workplace. It's also an excellent way to recognize the employees' safe work habits.
The Safety Awards presentation is held in conjunction with the annual FMA Safety Conference. The 2012 conference is scheduled for April 25-26 in Milwaukee.
The following awards are available to Advantage-level members only:
Safety Award of Honor
- Awarded to companies having perfect safety records of no recordable injuries or illnesses for the reporting period.
Safety Award of Merit
- Awarded to companies posting an injury and illness incidence rate for the reporting period that is better than the published Bureau of Labor Statistics (BLS) rate by 10 percent or greater, based on their NAICS code.
Safety Award Honorable Mention
- Awarded to companies posting an injury and illness incidence rate for the reporting period at least equal to, but less than 10 percent better, than the published BLS rate, based on their NAICS code.
Most Improved Safety Record Award
- Awarded to the member company posting an injury and illness incidence rate equal to or better than the published BLS rate, and also having the greatest percentage of improvement over the prior reporting year.
How to enter
- For your company to be eligible to receive a certificate of recognition, simply submit OSHA Form 300A, Summary of Work-Related Injuries and Illnesses. To receive a certificate, a perfect safety record is not required because the recognition is based on whether the company's incidence rate is better than the national BLS injury and illness incidence rate for that NAICS code. Companies fitting into one of the award categories listed above will receive a certificate and will be recognized publicly by FMA.
- The reporting period is Jan. 1, through Dec. 31, and the report must represent ONLY the division of your company at the location of your membership. Deadline for submission of the 2011 OSHA Form 300A will be Feb. 29, 2012.

Instructions for submission in the U.S.
- If you complete the fillable form from the links above, you may email the form to Nancy Hatten at nancyh@fmanet.org.
- You may also fax Form 300A along with a cover sheet that contains member name, member ID#, and the name of your facility's safety contact person to the attention of the FMA Safety Council at 815-381-1371. If you have any questions, please call 888-394-4362 or e-mail customer service.
Instructions for submission in Canada
- According to the Canadian Centre for Occupational Health & Safety and the Association of Workers' Compensation Boards of Canada, Canadian companies do not complete a form that is equivalent to the OSHA 300A Summary of Injuries & Illnesses, and the statistics for incidence rates by NAICS code or industry are not available for Canada on a national level.
- Since we cannot duplicate the Safety Award Program using Canadian information, we've adjusted the contest rules to allow Canadian companies to participate by completing and submitting the OSHA form in accordance with OSHA reporting guidelines. We will then compare it to the U.S. national average for the appropriate NAICS code.
- To be eligible for the Safety Awards Program, please become familiar with the OSHA requirements and start using the appropriate reporting forms.
NEW AWARD THIS YEAR
In addition to the above awards, this year FMA is pleased to introduce a new award:
The 2012 Rusty Demeules Award for Safety Excellence
This award was created by FMA's Safety Council to recognize companies that excel beyond OSHA recordable incidence rates — companies that have a demonstrated safety culture. Three awards will be given — one to a company of up to 25 employees, one to a company of 26 -100 employees, and one to a company with more than 100 employees.
The award is given in honor of Rusty Demeules, who started at Standard Iron and Wire Works in Monticello, Minn. as a machine operator, and worked his way up to president of the company founded by his father and three uncles. He worked at Standard Iron for 30 years, the last 10 of those as President and CEO, retiring in 1996.
Rusty made a significant contribution to FMA. He served on the FMA Board of Directors from 1983-1989, serving as Chairman of the Board from 1986-1987. He was a speaker at many FMA conferences. Rusty also served as President of the FMA/CNA Safety Committee from its inception in 1989 until 1996. One of his major accomplishments on the committee was the development of the FMA-sponsored insurance program, which offers insurance specifically designed to meet the needs of metal fabricating shops and tool and die makers. In 1989 Rusty received the FMA Award of Appreciation for his work with the organization, and in 1996 he received the FMA Chairman's Award for his long service to the industry and to FMA.
FMA is delighted to present this award in Rusty's name to companies that demonstrate that they have an excellent safety record and an established safety culture in the workplace.
