Award applications will be evaluated on the basis of the 8 criteria explained below. Please provide your application in the form of a Word document. Address each of the criteria, providing specific examples, or quantitative evidence as appropriate. If separate documents (such as letters from customers or OSHA 300A forms, etc.) are available to support your application, please include them.

Proprietary evidence will be kept confidential and used only in the judging process. If some evidence is only available as hard copy documents, not electronic files, you may fax your application as noted below.

Award Winners
Each winner will be selected by an awards committee consisting of staff and members of the Fabricators & Manufacturers Association and the Tube & Pipe Association respectively. Winners will be notified by Sept. 30, 2008.

Winners will receive:

The winner of The Fabricator Industry Award will appear on the cover of the February 2009 issue of The FABRICATOR and will be featured in a story appearing in the same issue.

The winner of the TPJ Industry Award will appear on the cover of the January/February 2009 issue of TPJ-The Tube & Pipe Journal and will be featured in a story appearing in the same issue.

The awards will be presented at a 2009 FMA Conference.
